Cemetery Disclosure Forms
The State Cemetery Board has created a series of cemetery disclosure forms as required by Chapter 20 of 2021. These documents have valuable information about your rights as a consumer. All cemeteries are now required to make these documents available to consumers. We have posted a QR code and a link to these documents at the cemetery so you may access them.
